What is Optimal Aging?
Optimal Aging is a partnership with Providence Health Plan which provides one on one in-home caregiver services to eligible Providence Health Plan members. We connect you with quality, professional caregivers who can assist you at home with non-medical services such as meals, mobility, housekeeping, exercise, medication reminders, bathing, companionship, transportation and more.
Who is it for?
Eligible Adult and Pediatric Providence Health Plan members can receive caregiver assistance when discharged from the hospital, during pregnancy and after birth, and support with mental health needs.
